CTU Wounded Warrior Scholarship Application: Required Documents
As part of the Wounded Warrior Scholarship application, we ask for proof of injury. Your proof of injury statement does not need to contain details regarding the injury, however should verify that the individual was injured while in the line of duty.
For Active Duty and Veteran applicants, acceptable documents include:
- A letter from the U.S. Department of Veterans Affairs
- A physician’s statement
- A statement from a Commanding Officer
For Spouse and Non-Medical Attendant applicants, acceptable documents include:
- A marriage certificate
- A letter from the U.S. Department of Veterans Affairs OR
- A letter from the U.S. Department of Defense stating that you are the primary caregiver for the service member
If you are a spouse or non-medical attendant, you’ll also need to upload the injured service member’s DD Form 214, or DD214. A DD214 is issued by the Department of Defense to each veteran, identifying the condition of discharge. This document is often used when applying for benefits, retirement, employment and membership to veterans’ organizations.
